About this role
## Principal Software Engineer ### About the Role Help design, build, and operate Vytalize Health’s clinical decision support and API-driven data delivery platform. You’ll work cross-functionally to deliver timely, actionable clinical insights directly into provider workflows—moving from external tools to seamless in-workflow decision support via **CDS Hooks** and **real-time APIs**. You’ll own the technical foundation for **Clinical Decision Support (CDS) Hooks**, **REST APIs**, and the architectural patterns that ensure clinical data is accessible, trustworthy, and performant at the moment of care. ### Primary Responsibilities - Lead design and implementation of the **CDS Hooks platform** (card orchestration, hook context handling, EHR-side logic orchestration, downstream data service integration) - Own **CDS Hook API contracts** and their evolution, ensuring compliance with the **SMART CDS Hooks** specification while improving performance and reliability - Integrate with **EMR systems** (Epic, NextGen, Athena, ECW) for CDS Hook discovery, execution, card rendering, and user action handling - Design, build, and maintain **public-facing REST and event-driven APIs** exposing clinical data, quality metrics, care gaps, attribution data, and decision support content - Establish **API contracts**, versioning strategies, and **deprecation policies** for external consumers - Implement API security patterns including **OAuth 2.0**, scope-based authorization, rate limiting, and **audit logging** - Design and optimize **real-time / near-real-time data pipelines** feeding CDS Hooks and APIs (low latency, high freshness, graceful failure modes) - Build comprehensive **testing and QA** frameworks for CDS Hooks and APIs (context validation, card rendering verification, workflow testing across EMRs) - Define and measure **reliability metrics and SLAs** (latency, availability, freshness) for CDS Hook delivery and API performance - Define and measure **clinical impact metrics** (adoption, interaction rates, workflow integration success, patient outcome influence) - Apply architectural judgment across correctness, latency, scalability, maintainability, and operational risk - Implement reliability patterns (monitoring, alerting, failure detection, graceful degradation, incident recovery) - Partner with Data Reliability Engineering to define and operate SLAs aligned to clinical needs - Uphold engineering standards for API design, data schemas, integration patterns, and interoperability; maintain strong technical documentation and **OpenAPI** specs - Partner with Data Engineering to align upstream data models and datasets with clinical decision support and API requirements - Serve as a technical authority on clinical interoperability standards (SMART CDS Hooks, FHIR, HL7, EHR integration patterns) - Collaborate with Product, Clinical Operations, and business stakeholders to translate clinical workflows into platform capabilities - Ensure compliance with healthcare security, privacy, and regulatory requirements (e.g., **HIPAA** and state regulations) - Mentor teammates through design reviews, pairing, and architectural guidance - Document systems, designs, processes, and procedures ### Required Qualifications - **8+ years** designing, building, and operating API platforms, service-oriented systems, or backend infrastructure at scale - Senior/staff-level experience owning complex systems end-to-end - **3+ years** healthcare data integration experience (E
