We help clinicians launch and scale their own clinics by providing **AI-powered software**, **payer contracting**, and **full back-office infrastructure**. This role is **full-time**. **Location / Work style:** Based in **New York City** or **San Francisco** with a **5 days in-office** culture. --- 🎯 **ROLE: SENIOR BACKEND SOFTWARE ENGINEER** We’re a tiny engineering team building something large. You’ll be the person who **owns the platform foundation**—including **data models, money logic, state machines, API contracts, and AI infrastructure**. This is not “owning” in the sense of reviewing PRs. It’s owning in the sense that **you designed it, you built it, and you stand behind it**. You’ll work across high-impact problems—from **schema migrations** to **LLM pipelines for clinical documentation**, to **debugging billing edge cases** (e.g., payer interactions like Colorado Medicaid as a secondary payer). --- 🚧 **WHAT YOU’LL OWN** - **Core Platform Integrations** - Build and maintain critical integrations with vendors, clearinghouses, and healthcare systems (e.g., **Candid, Silna, Availity, HubSpot**, payroll providers, and **state Medicaid systems**). - Ensure data flows reliably across the Alpaca platform. - **Revenue & Operations Infrastructure** - Own backend systems powering **billing, authorizations, eligibility, patient responsibility, payroll, and provider payouts**. - Design engines that move millions of dollars with **accuracy, auditability, and trust**. - **Internal Systems & Automation** - Replace spreadsheet/PDF/inbox workflows with software and **AI-powered automation** for credentialing, authorizations, billing operations, compliance, and care coordination. - **Data & Reporting Platform** - Build the data infrastructure that powers reporting across the company. - Own pipelines, models, and backend services that give providers visibility and help internal teams make operational decisions. - **Payroll & Workforce Systems** - Design systems for provider compensation, payroll integrations, time tracking, PTO, and workforce operations. - Help make getting paid feel seamless while ensuring compliance and financial accuracy. - **Platform Foundations** - Own underlying systems: **APIs, data models, event-driven architectures, security, compliance, reliability, and AI infrastructure**. - Build systems that remain correct as Alpaca scales from **hundreds to thousands** of providers. --- 🧠**WHO YOU ARE** - You’ve built in domains where mistakes matter (e.g., **fintech, healthcare, insurance, logistics**), with a focus on **data integrity, money movement, and business rules**. - You think in **systems**: model states, transitions, invariants, and edge cases. - You have strong **product instincts**: you care about user experience and can translate provider pain points into simple software. - You thrive in **ambiguity**: you can define the right questions and drive execution without a fully written PRD. - You operate with **high ownership**: you don’t just write tickets—you make blockers disappear. - You’re **AI-native**: you use AI daily and experiment with ways to incre
